Otherwise, download a membership application or a Friend of the College application. Then fax the completed form (front and back) to 240-485-1818 or mail it to the address on the form.

Active Member Full active membership in the College open to any CNM or CM. Membership includes voting privileges and the ability to list your Practice on Find-A-Midwife. Dues: $335	Associate Member CNMs/CMs only: full-time student/disabled/retired/missionary/full-time volunteer/non employed Dues: $130
Active - First Year For CNMs/CMs certified during the last year. Dues: $205	Students For students enrolled in ACNM accredited midwifery education programs. Dues: $130
Active - Retired/Disabled Must have been a member for ten years. Dues: $205	Friend of College This affiliation program allows persons interested in midwifery and ACNM to receive the Journal of Midwifery & Women's Health and Quickening. Dues: $120
Active - Life Lifelong membership in ACNM. Dues: $5025
