Overview

The Find a Midwife practice locator is a web-based service that allows health care consumers to find midwifery practices in their area, and supply them with basic contact information like practice name, address, phone number, e-mail address, web site and a map of the area. There is no charge for this listing!

Requirements for Inclusion

Listing in the Find a Midwife practice locator is open to all practices with at least one ACNM-member CNM or CM on staff.

To Add Your Practice (or update practice information):

1. Click here to download the Practice Update form. This form requires Acrobat Reader.
2. Fill out and fax this form to the ACNM at (240) 485-1818, or mail to:

3. The practice database is updated bi-weekly, so be patient while your information is added to the database!
