Scholarship and Award Reviewers

Dear Valued Colleague,

I thank you again for taking the time to complete The A.C.N.M. Foundation, Inc. Reviewer Questionnaire, which is part of our recent effort to expand the pool of experienced reviewers for our growing scholarship and awards programs. Before you begin, I want to assure you that the information you provide will not be used for any purpose other than to assist us with the selection of qualified and experienced reviewers.

In the following survey you will find several questions about your professional background and experience that will help us match potential reviewers with our various applications, whether they be related to education, research, policy, leadership or global health and to help us avoid obvious conflicts of interest.

You will be asked to answer optional demographic questions at the beginning of the survey. Your answers to these questions will enable us to assemble diverse review groups best suited to a given scholarship or award. You will have the option to choose whether or not to answer all survey questions.

Lastly, I want to assure you that prior to any request for your volunteer review time, you can expect to be fully informed about your role as well as the estimated time commitment involved. You might also be asked at that time to provide more details about your background and experience as we determine the optimal reviewers for each scholarship or award.

I thank you again most sincerely,

Elaine M. Moore, CNM, MSN, FACNM

President
